Living Will kits containing the legal forms and instructions can be purchased for a small fee from the Ohio State Medical Association at P.O. Box 931, Columbus, Ohio 43216-0931. The small fee covers printing and mailing. Here are some of the main provisions of the Living Will according to the Ohio State Bar Association:
- A binding legal document that declares your wishes for using life-sustaining measures.
- Takes effect only when two licensed physicians determine you are permanently unconscious or terminally ill and unable to communicate.
- Spells out whether you want artificial life support equipment.
- Requires doctors to follow your instructions regarding treatment, including possibly withholding internal feeding and fluids.
- Cannot be revoked by anyone but you. You may revoke or change it at any time.
