Living Will kits containing the legal forms and instructions can be purchased for a small fee from the Ohio State Medical Association at P.O. Box 931, Columbus, Ohio 43216-0931. The small fee covers printing and mailing. Here are some of the main provisions of the Living Will according to the Ohio State Bar Association:

1. A binding legal document that declares your wishes for using life-sustaining measures.
2. Takes effect only when two licensed physicians determine you are permanently unconscious or terminally ill and unable to communicate.
3. Spells out whether you want artificial life support equipment.
4. Requires doctors to follow your instructions regarding treatment, including possibly withholding internal feeding and fluids.
5. Cannot be revoked by anyone but you. You may revoke or change it at any time.
