Gift of Life Plan*

Life plan for children ages 0 to 18. For some, the offer of conversion privileges is important should the child's health deteriorate; for others, these plans provide low cost insurance at an early age to cover the risk of loss. For still others, this is an important step in assisting their child in starting their own plan to cover them throughout life. These plans offer life benefits of $15,000 and $30,000 with a flat rate for ages 0 to 18.  Contact us for a review and quote for your child. 

If you are a college student, Student Select may be for you! Student Select is renewable individual health insurance designed for today's undergraduate and graduate college students. This plan is ideal if you're a single student no longer covered under your parents' health plan, and if you live outside your parents' HMO or PPO area. Click the download to review and print.  The application must be completed, signed and mailed (with your payment made payable to Fortis Health Insurance Company) to our office for processing.
