Baptismal Certificate/Release Form Suggestion
This form can be quite simple in nature and should be generally used for any children 18 years and younger. It should clearly state at least the following:
What the baptismal service entails - its purpose and significance in the life of the child.
How the service and the actual baptism is performed so that an informed parent(s) or guardian can make an informed choice to support the child's decision to be baptized.
Encourage the parents(s) or guardian to be present and in so doing show their support for the child.
Provide for the signature and date of all parent(s) or guardian.
The form should be kept on file indefinitely. A copy should also be given to the parent(s) or guardian.
Include ministry contacts should the parent(s) or guardian have any questions.
Some churches require that all candidates for baptism attend an informational class and for children age 18 and under strongly encourage their parent(s) or guardian to attend with them so that there is less likelihood for misunderstandings.
